Initial Appointments: Most new patients require a physician referral to see one of our physicians. Your personal physician or surgeon will provide records of your medical condition and previous medical care. We also welcome the opportunity to provide second opinions. Please call our office for more information.

We accept most insurance plans and our staff will work with you to make sure that you have a thorough understanding of your coverage.

Please plan to arrive at our office at least 15 minutes prior to your scheduled time so that you can complete new patient forms, the receptionist can obtain any additional information that may be needed for your appointment, and can copy your insurance cards.

At every appointment: We do our very best to accommodate your scheduling needs. Please honor your scheduled appointment time. If you need to cancel an appointment, we request that you provide as much notice as possible.

We strive to keep waiting times to a minimum, but, unfortunately, delays can occur due to emergencies and unforeseen patient needs.

We ask that you always:

  • Write down any questions or concerns that you may have to discuss with your physician
  • Bring a list of all medications that you are currently taking (including over-the-counter remedies, vitamins and herbal supplements)
  • Bring your insurance cards
  • Let us know whenever you change your address, telephone number, employment, marital status or insurance
  • You may bring someone with you to help listen to your physician’s comments and responses to your questions
  • Make your next appointment every time you visit the clinic, including lab, treatment and radiology appointments
